What is The Hudson Community Project: Safe at Home?
The program connects older adults with services that help them maintain autonomy by making living at home more comfortable and secure. The Hudson Community Project is a partnership among the YM&YWHA of Washington Heights & Inwood, the Jewish Community Council of Washington Heights-Inwood, the Riverdale YM-YWHA, and the Hebrew Home at Riverdale. By combining the resources from these agencies, we are able to expand the geographic scope of our services and to streamline the way we provide them. The result: older adults can receive a more comprehensive selection of services aimed at helping you maintain your independence and make living at home more comfortable and secure.

How much does it cost?
Almost all our services are free! There may be a nominal fee associated with some transportation services. In-Home Support services are $12/hour.

Who is eligible?
Participants must be 60 or older and must not be a recipient of Medicaid. A home assessment is required.

All you have to do is pick up the phone! Call us at 646-738-6092 to get started. Our professionals will assess your requirements and develop an ongoing care plan so you have access to the support you need.

What are examples of The Hudson Community Project’s services?
Everyone’s situation is unique and each individual’s care plan will reflect his or her specific needs. These are some examples of what a service package may include:

Case Management: Physical and mental health assessment; Personalized care plan; Referrals for social adult day care; Ongoing support

Counseling: Legal; Financial; Spiritual

In-Home Support: Light housekeeping; Laundry; Shopping; Personal care

Transportation: Riverdale area: free wheelchair-accessible vehicle to/from the Riverdale Y; Washington Heights/Inwood area: free wheelchair-accessible vehicle to/from the Washington Heights Y; Washington Heights/Inwood area: $2 rides to doctor appointments, the bank, senior centers, etc.

Kosher Meals: Shabbat and holidays meals available for pickup and delivery; Daily meals at the Washington Heights and Riverdale Ys; Monthly kosher pantry of non-perishable grocery items

Housing Advocacy: Landlord-tenant advocacy; Housing-related entitlements; Minor repairs

Rehabilitation: Priority access to rehabilitation services; Short- and long-term residential care
